You will be emailed a set of questionnaires before the consult, or you will fill them out at time of consult (doing them prior to your appointment will allow for richer discussion on your first appointment). Dr. Purtzki will take a comprehensive history and perform a complete physical examination. Thereafter she will provide you with a verbal opinion, followed by a written report with recommendations. Together through collaborative discussion, we determine the needs and identify if a team meeting with community rehabilitation team is appropriate.

The following is the core rehab team:

  • OT, PT (physio) rehab assistant, psychologist/councillor,
  • Doctor,
  • teacher,
  • recreation therapist,
  • Neuropsychologist,

Additional specialists may be appropriate on a case-by-case basis.

When patients experience brain injury or trauma, their lives aren’t the only ones disrupted. To recover, they require support from family and loved ones. But what do the loved ones need in order to help? It’s important to assess not only patients’ needs but also the needs and abilities of those supporting them to ensure the right resources are in place and contributing toward successful outcomes. Family-centred approach begins with the patient at the centre and their family and loved ones around them with their needs addressed so they can contribute to a successful health outcome.

Assessments are done on a case-by-case basis to determine what is covered by MSP.

Mondays and Wednesdays 9:30am – 5pm

Usually two to two and half hours

Gym shorts, tank top (for adolescents), any prior consultations reports, imaging reports, psycho educational assessments, recent therapy reports, school records if relevant.

Fees are individualized based on needs please contact for a quote.

Dr. Purtzki will screen each referral individually. A written referral is required.
